Welcome to our Patient Portal!

We now use electronic medical records, which enable you to input your information prior to your appointment, and will save you from having to fill out forms once you arrive. In the Patient Portal you will enter your demographics, insurance (for ENT patients) and medical history.

Please read below. Once logged into the patient portal, follow these simple steps. Click the link to the right to log in:

STEP 1: Click on the Demographics link to review your contact information. Please verify that everything has been entered correctly and then click the “Save” button.

STEP 2: If you are an ENT patient click on the Insurance link and fill in your information. Please have your insurance card with you when you arrive at our office. If you are a COSMETIC patient you may skip this step.

STEP 3: Click on the Forms link to be taken to the online medical history form. Click on the form listed to open it. Fill out each section, using the “next” button to access the next section. If you have to stop before finishing, click on “Save” to save your changes. You can continue editing the form at any time by reselecting it from the Forms page.
NOTE: If you are NOT finished with the medical history and need to save and complete at another time. DO NOT click “Continue to Final Step”, click the “Save and Resume Later”. You will be able to return at another time to update and/or complete. Please keep your username and password available for return access.

STEP 4: When you are finished filling out the Medical History Form, click the “Continue to Final Step” button. Click the “Submit” button to complete the form. Once submitted, your medical history form will no longer be available for editing or viewing.

STEP 5: Click “log out”
